This includes Medicare Replacements such as UPMC for Life, Humana, Security Blue, Freedom Blue, etc.

This includes PPO Blue, Keystone Blue, Capitol Blue Cross, Indemnity BC/BS, PEBTF, etc.

 

4. UPMC

This includes UPMC for Life, UPMC for You, UPMC for Kids Chip.

 

5. Medical Assistance (Access)

 

6. American Specialty Health Network (ASHN)

This includes some Geisinger Health Plans which must state American Specialty Health Network on the back of their insurance card. ​
